Duplicate files of the same title become one catalog row, with one stream per member file.

fix: skip torrent folders named like videos so catalog sizes and streams resolve d968fb7037 
NFS/WebDAV listings treat a TorBox folder named *.mkv as a 0-byte video. Scan now keeps real files only, grouping drops size-0 placeholders, and stream resolve uses the index instead of a full TMDB catalog walk.
fix: memoize index-backed stream grouping ceba36fd00 
Avoid rebuilding the 7k-file grouped catalog on every POST /v1/stream after the first resolve.
